(4) Öğretim üyesi/elemanının görev aldığı tüm programlardaki ders yükünün haftalık toplamı yazılmalıdır.

(5) Programın öğretim dilinin yabancı dilde veya en az %30 yabancı dilde olduğunda öğretim üyelerinin/elemanlarının yabancı dil belgelerine ilişkin bilgilerin buraya yazılması gerekmektedir. Örneğin: 2006 KPDS Aralık - 95 gibi
