DAY-30 & 31
FISHBONE ANALYSIS
Fishbone diagram
The cause & effect diagram is the brainchild of Kaoru Ishikawa, who pioneered quality management processes in the Kawasaki shipyards. This analysis tool organizes a large amount of information by showing links between events and their potential or actual causes and provides a means of generating ideas about why the problem is occurring and possible effects of that cause. Cause-and-effect analyses allow problem solvers to broaden their thinking and look at the overall picture of a problem. Cause-and-effect diagrams can reflect either causes that block the way to the desired state or helpful factors needed to reach the desired state.
The head of the fish shows the effect the bones show the cause of the problem
When should a fishbone diagram be used?
DAY-30 & 31
-
Need to study a problem/issue to determine the root cause?
-
Want to study all the possible reasons why a process is beginning to have difficulties, problems, or breakdowns?
-
Need to identify areas for data collection?
-
Want to study why a process is not performing properly or producing the desired
results?
How is a fishbone diagram constructed?
-
Agree on a problem statement (effect)
-
Draw the fishbone diagram....
-
List the problem/issue to be studied in the "head of the fish".
-
Label each ""bone" of the "fish". The major categories typically utilized are:
The 4 M’s:
Methods, Machines, Materials, Manpower
The 4 P’s:
Place, Procedure, People, Policies
The 4 S’s:
Surroundings, Suppliers, Systems, Skills
DAY-30 & 31
-
Brainstorm potential causes of the problem
Brainstorm all the possible causes of the problem. Ask “Why does this happen?”
As each idea is given, the facilitator writes it as a sub-cause branching from the appropriate main cause. Sub-causes can be written in several places if there are multiple relationships.
6. Review each major cause category. Circle the most likely causes on the diagram
7. Asking "why" will help get to the root cause of the problem.
Write sub-sub-causes branching off the sub-causes. Continue to ask “Why?” and
generate deeper levels of causes. Layers of branches indicate causal relationships.
DAY-30 & 31
The enclosed diagram explains how a fishbone diagram is constructed.

DAY-33
Present moment awareness
Pair A
The mind is often our own _________________, controlling us and filling us with ____________________________________. You are not your mind, ________________ ____________________.The constant chatter that goes on in most of us is usually related to either the past or the future and rarely to ______________________. Not being able to stop thinking is a dreadful affliction, but we don't realise this because _____________________ ______________, so it is considered normal (like _________________________________
_______________). If we are constantly living in the past or the future, then none of us is
really here in the present moment and we_______________________________________. Incessant mental noise prevents you from finding _______________________________ that exists in all of us and that is inseparable from the Divine Being, _____________________.
If you think about all the stresses and worries that you have in your life now, ask yourself
How many of these actually relate to the present moment. For most of us, ___________________ stems for some future points in time that will never arrive, or
_________________________ that can never be repeated. We always expect better things to come in the future and we are _________________________________ of the present moment, ___________________where happiness is possible. If we are all just living for some _______________________, it will never come. The only time we can be truly happy in ___________________________. What is stopping you? Your mind.

Appendix VI
Present moment awareness
Pair B
_______________ is often our own worst enemy, ______________and filling us with
worry, stress and negativity. ________________________________, but your mind would like you to think so. The constant chatter that goes on in most of us is usually related to either ____ _______________________ and rarely to the present moment. Not being able to stop thinking is _________________________, but we don't realise this because almost everybody is suffering from it, so it is considered _______________ (like a diet of cooked and processed foods). If we are constantly living in the past or the future, then none of us is really here in the present moment and we cannot say that we are truly alive. _____________ __________________ prevents you from finding the inner peace and stillness that exists in all of us and that is inseparable from __________________, the Eternal Source. If you think about all the stresses and worries that you have in your life now, ask yourself how many of these actually _____________________________________. For most of us, all of our negativity __________________________________________ that will never arrive, or from some past events that can never be repeated. We always _______________________ to come in the future and we are never really fully appreciative of ___________________, the only moment __________________________________. If we are all just living for some
future happiness, ______________________. The only time we can be truly happy in is
right here and now. What is stopping you? __________________.
