Abstract: The large prospective studies on adverse effects of oral contraceptives have unanimously revealed an increased risk of thromboembolic diseases, which seem to be associated with the dose of ethinylestradiol (EE). According to the recommendations of several medical committees, the dose of EE has, therefore, been more and more reduced; in some countries there are now ovulation inhibitors containing 20-mu-g EE. Since serious reactions, which have a relatively low incidence, are highly underreported (less than 10 %), it is difficult to prove dose-dependent differences in the rates of cardiovascular diseases. There is, however, virtually no doubt that not only the incidence of thromboembolic diseases and stroke, but also that of benign liver tumours and gall bladder diseases is increased in relation to the EE-dose. A series of metabolic serum parameters, e.g. serum binding proteins, coagulation and fibrinolysis factors, angiotensinogen, is changed by EE in a dose-dependent manner which is, however, limited when the effects are receptor-mediated. Higher doses of EE have been shown to facilitate fibrin deposits on vascular subendothelium. The pharmacological effects of EE are to a large extent dependent on the dose, e.g. the irreversible reactions of EE and other ethinylated steroids with hepatic enzymes which are involved in the metabolism of steroids, drugs and toxic compounds. After long-term treatment with combinations containing 50-mu-g EE, in half of the women, abnormal liver function tests with pathological morphological alterations have been found. As combinations with low EE doses and a sufficiently effective progestogen component do not differ from higher dosed oral contraceptives in their contractives safety and cycle control, there are no indications for pills containing 50-mu-g EE, except the normophasic sequential preparations for women with sustained irregular bleedings when taking low dose combinations

Abstract: Thrombolytic therapy has become an established procedure in patients with acute myocardial infarction (AMI). However, the aftercare of such patients is still uncertain. A meta-analysis of twenty trials of intravenous or subcutaneous heparin in AMI performed during the prethrombolysis period indicated a significant reduction in mortality, reinfarction, and stroke in treated patients. More recently, a study (SCATI) aimed at investigating the clinical effects of subcutaneous heparin (12,500 U two times daily) in the setting of thrombolytic therapeutical strategy in AMI, showed a lower in-hospital mortality, a trend towards lesser transient ischemic episodes in patients given streptokinase, and no difference in recurrent infarction rate. Ventricular thrombi were markedly reduced by heparin. Anti-platelet drugs were not permitted in the SCATI. In both GISSI 2 (Gruppo Italiano per lo Studio della Streptochinasi nell'Infarto Miocardico) and International tissue plasminogen activator/streptokinase trials, aspirin was a recommended treatment, and subcutaneous heparin was randomized. No difference in mortality was noted in patients given heparin. In GISSI 2 recurrent ischemic episodes were also similar in treated and control groups, whereas embolic events were reduced by heparin. Major bleedings were rare in all trials. In conclusion, subcutaneous heparin is beneficial in AMI; however, the association with aspirin does not add consistent benefits. Heparin is effective in preventing thrombus formation and embolic complications - such effects are not shared by aspirin

Abstract: Patients surviving acute myocardial infarction (AMI) may experience several clinical events (reinfarction, congestive heart failure, sudden death) still responsible for high mortality rates. AMI early complicated by residual angina, left ventricular dysfunction, or malignant arrhythmias has a worse prognosis. Secondary prevention of myocardial infarction and death has been the end point of many clinical trials in the past two decades. It is well known that beta blockers prevent sudden death if administered chronically after AMI. Meta- analysis of controlled randomized trials demonstrated a significant reduction in reinfarction and vascular death with long-term antiplatelet treatment. Oral anticoagulants prevent fatal and non-fatal reinfarction and show a trend towards lower mortality rates, though treated patients have a higher incidence of haemorrhagic events, particularly stroke. Early administration of heparin gave contradictory results on short- term prevention of myocardial infarction and death after AMI. Data on long-term heparin therapy point out a significant reduction in recurrent AMI and a trend towards a decrease in general mortality

Abstract: The pathogenesis of thrombosis involves consideration of two processes: atherosclerosis and thrombosis. Atherosclerosis is a prolonged process in which interaction of lipids and the haemostatic system produces focal lesions at sites of turbulence in arteries. These lesions may rupture, precipitating the complication of thrombosis and leading to complete occlusion of the vessel, causing myocardial infarction, stroke, or acute ischaemia. In veins the atherosclerotic process does not occur, and thrombosis is precipitated by immobility often associated with accidental or surgical trauma. The changes in platelets, coagulation, fibrinolytic systems leading to atherosclerosis and thrombi are reviewed. The role of platelets and coagulation changes in these processes are emphasized by the beneficial effects of therapy, both antiplatelet and anticoagulant therapy. Last, the concept of hypercoagulability leading to accelerated arterial disease and the clinical value of laboratory tests used in its detection are considered

Abstract: A 30-year-old woman with a history of common migraine developed a permanent left homonymous hemianopia during a typical headache. CT scan demonstrated a right posterior cerebral infarction and angiography showed irregular narrowing of the ipsilateral posterior cerebral artery, suggestive of vasospasm. Abstract: Intraluminal clot of the internal cervical carotid artery is commonly thought to require emergency surgery: 7 intraluminal clots specially threatening (6 of whom had a long defect - 15 mm and more) are demonstrated by intraarterial digital angiography - 4 patients experienced mild stroke, 3 major stroke. 3 of whom had previous recurrent T.I.A. (3 transient blindness, 1 hemispheric TIA). Carotid angiography identified 3 severe atherosclerotic stenosis, 3 ulcerated plaques and 1 dissection. One patient with coma carus died quickly. Anticoagulation therapy (6 cases) was made, 4 weeks along, without neurologic complications. Follow-up angiograms showed total resolution (4 cases), partial lysis (1 case) and mild extension (1 case). Delayed endarterectomy was made only for severe carotid atheroclerosis (5 cases). In our experience, intraluminal clot of the carotid artery may not be a surgical emergency but require anticoagulant therapy and delayed surgical if major underlying lesions
