Abstract: The relative hemodynamic profile between dual chamber pacing (DDD) and activity sensing rate responsive pacing (VVIR) was compared in ten patients with dual chamber rate responsive pacemakers (Synergist II). With a double blind, randomized exercise protocol, DDDR pacemakers were programmed into VVI, VVIR, and DDD (AV interval 150 msec) modes and in seven patients the test in the DDD mode was repeated with the AV interval programmed at 75 msec. A treadmill exercise test of 6-minutes duration (2 stages, Stage I at 2 mph, 0% gradient and Stage II at 2 mph, 15% gradient) was performed at each of the programmed settings, with a rest period of 30 minutes in between tests. Cardiac output was assessed using continuous- wave Doppler sampling ascending aortic flow and expressed as a percentage of the value achieved during VVI pacing. During exercise, pacing rate between DDD and VVIR pacing was similar but was higher with DDD at the first minute of recovery (91 +/- 4 vs 81 +/- 3 beats/min, respectively). Cardiac output was significantly higher at rest, during low level exercise, and recovery with DDD pacing compared with VVIR pacing (resting: 21 +/- 14 vs -2 +/- 7%; Stage I: 36 +/- 6 vs 16 +/- 7%; Stage II: 25 +/- 15 vs 10 +/- 8%; recovery: 26 +/- 12 vs 4 +/- 9%; P < 0.05 in all cases). Systolic blood pressure was significantly higher during low level of exercise in the DDD mode. Shortening of the AV interval to 75 msec did not significantly affect cardiac output during exercise, but cardiac output after exercise was reduced (2 +/- 6 vs 23 +/- 6% at an AV interval of 150 msec, P < 0.02). By enhancing the stroke volume, DDD pacing improves cardiac hemodynamics at rest, during low level exercise, and early postexercise recovery

Abstract: This study was designed to investigate the haemodynamic effects of d-sotalol at plasma concentrations producing class III antiarrhythmic effects. d-Sotalol 1, 4 and 10 mg/kg intravenously was given after beta-blockade (propranolol 0.25 mg/kg intravenously) to seven pentobarbital anaesthetized dogs. Left ventricular (LV) systolic and end-diastolic pressures, LV dP/dt(max), mean aortic pressure, stroke volume, cardiac output and total peripheral resistance were not significantly changed by d-sotalol. There was a linear correlation between the dose of d-sotalol infused and the plasma concentration of d-sotalol obtained. Heart rate decreased and QT-time increased with increasing plasma concentrations of d-sotalol, whereas the QRS- width did not change. There was a linear correlation between the decrease in heart rate and the increase in QT-time, and between the plasma concentration of d-sotalol and increase in QT-time. In conclusion, the study indicates that after beta- blockade d-sotalol has no cardiodepressive effects at concentrations that prolong repolarization

Abstract: The functional consequences of cortical focal ischemia and the effect of monosialoganglioside (GM1) treatment on learning/performance of a spatial reversal task were investigated. Cortical focal ischemia was induced by a permanent occlusion of the left common carotid artery and the ipsilateral middle cerebral artery, with a 1-h clamping of the contralateral carotid artery. Twenty-six rats were randomly assigned to three groups: sham controls, a saline-treated ischemic group, and a GM1 ganglioside-treated ischemic group (10 mg/kg/day: IM). Fifteen days after surgery rats were trained on a spatial reversal task in a two-lever operant chamber where food reward was contingent on lever pressing. Training continued from day 15 to day 21 after surgery. Cortical focal ischemia resulted in learning/performance deficits that were reduced by GM1 ganglioside treatment. The cognitive deficits were characterized by a significantly higher number of nonperseverative errors and number of responses to criterion. There was a significant difference between left and right lever performance in the saline-treated ischemic group, which was absent in shams and GM1-treated ischemic rats. On all measures GM1-treated rats were not different from sham controls

Abstract: The cardiovascular effects of intravenous administration of ketorolac tromethamine 10 and 90 mg were measured and compared with those of morphine sulfate 10 mg in 18 anesthetized patients after major vascular surgery. Morphine caused a significant reduction in mean arterial pressure, left cardiac work index, and left ventricular stroke work index. Stroke volume index, cardiac output, and pulmonary artery pressures decreased to a lesser extent. Ketorolac at either dose was not associated with any significant change in cardiac and hemodynamic parameters. The data suggest that even high doses of the drug may be safely administered in clinical situations where hypotension is disadvantageous or where reduction in afterload is not a specific therapeutic aim

Abstract: Two young patients with sickle cell trait (AS haemaglobinopathy) and ischaemic stroke are reported. The stroke involved the internal carotid artery territory in one and the brainstem in the other. A review of the literature is presented to suggest that the association of sickle cell trait and cerebral infarction is more than coincidental. Haemoglobin electrophoresis should be undertaken routinely in young subjects with ischaemic stroke

Abstract: Septic shock (SS) is characterized by initial hyperdynamic hemodinamic profile followed by hypodynamic state. This last state includes multiple factors in its origin. In non hyperdynamic SS the predictive value of hemodynacim parameters (HP) are discussed. We analized in 40 septic patients (P) without hyperdynamic status (cardiac - index [Cl] < 3 1/min/m2) and without hypovolemia (wedg pressure (WP) > 12 Torr). Baseline's HP (heart and respiratory rates, mean arterial pressure, mean pulmonary pressure, WP Cl, work stroke index, systemic vascular resistance, pulmonary vascular resistance [PVR], arterio-venous 02 difference; oxigen consumption and minuted diuresis) and its relationship with vital evolution. Mortality was 77.5 %. We considered two groups of P: either survivals (S) or not survivals (NS), from baseline HP only PVR was significantly different between both (p < .01) (greater in NS). Cl was greater in S than NS, if we considered those with similar WP value (> 12 Torr). The conclusions are: - High mortality of non hyperdynamic state sepsis. - Usefulness of PVR as predictive index in septic patients with low Cl and without hypovolemia. - Possible development of highen right ventricular disfunction secundary to changes in the pulmonary vascular bed
