Abstract: Failure of rapid adjustment of stroke volume and cardiac output compensating for the sluggish response of heart rate in heart transplant recipients (HTR) during exercise has been suggested to restrict the individual's physical performance. To test this hypothesis, impedance cardiography was used to study the steady-state and transient responses of heart rate (HR), stroke index (SI), cardiac index (CI), left ventricular ejection time (LVET) and pre-ejection period (PEP) during upright cycloergometric exercise (50 W, 5 min) in 21 short-term (less than 1 year after operation) and long-term HTR (age 44 +/- 8 years; 23 +/- 29 months after transplantation, range 1.3-137 months) and 10 normal healthy subjects (CTL, age 37 +/- 10 years). Comparison between the two groups at rest (means +/- SD) revealed higher HR (102 +/- 12 vs 72 +/- 9 beats/min), lower SI (39.7 +/- 15.2 vs 49.5 +/- 10.3 ml/m2, p < 0.05) and LVET (218 +/- 21 vs 255 +/- 22 msec, p < 0.01) in HTR while CI (3.98 +/- 3.58 vs 3.58 +/- 1.00 L/[min.m2], p > 0.05) and PEP (92 +/- 21 vs 83 +/- 30 msec, p > 0.05) were similar. During exercise SI was similar in both groups (72.6 +/- 23.4 vs 71.6 +/- 25.8 ml/m2, p > 0.05) but CI was significantly higher in HTR than in CTL (8.71 +/- 3.16 vs 6.73 +/- 1.92 L/[min.m2], p < 0.05) while HR had approached different levels (120 +/- 12 vs 98 +/- 16 beats/min, p < 0.01). Similar levels of HR, SI and CI were achieved by HTR and CTL upon repetitive 50 W exercise after 5 min recovery from the first test despite HR in HTR had remained elevated. In both groups the increased SI was associated with a marked decrease of PEP (34%) and moderate decline of LVET (8%). The half-time of the 'on'-response of cardiac output, defined as the time required to reach 50% of the asymptotic steady state or peak level, was only moderately delayed in HTR compared with CTL (56 +/- 19 vs 37 +/- 15 sec, p < 0.01) and a similar tendency was observed for the 'off'- response (44 +/- 20 vs 33 +/- 19 sec, p > 0.05). Similar dynamics were observed during repetitive 50 W testing after 5 min recovery. It is concluded that the transplanted denervated heart, by virtue of augmented preload and the Frank-Starling mechanism, achieves a normal (actually slightly enhanced) cardiac output during moderate dynamic exercise. The insignificantly retarded kinetics of the 'on'-response of cardiac output is attributed to delayed onset of venous return and would seem to suggest a 'peripheral' rather than a 'central' mechanism
