Abstract: Based on the phase difference method as described by Nayler et al., we developed a gradient-echo sequence, which refocuses flow-related phase shifts. With regard to the higher peak velocity and higher acceleration in infants, we reduced the echo time (TE) to 5 ms. This is effective in rephasing the flow signals even for faster heart rates. Phase shifts are further minimized by reducing the voxel size. The slice thickness down to 2 mm also improves anatomic resolution. A repetition time (TR) of 15 ms provides high temporal resolution for dynamic studies. Modification of the flow-rephasing gradient-echo sequence allows blood flow measurements in the great arteries and the calculation of blood flow volume to assess left and right ventricular stroke volume. This can also be achieved by calculating the ventricular volume from contiguous slices of the whole heart; however, it results in excessive measuring times. Compared with conventional spin- echo techniques, anatomical analysis is improved. The size of atrial septal defects (ASD) can be measured more exactly. Small atrial or ventricular septal defects (VSD), which are not detectable on spin-echo images, are demonstrated on the gradient-echo images and semi-quantitative estimation of the pressure in the right and left ventricle can be made. Pulmonary arteries and veins are clearly differentiated from bronchial structures and are traced to subsegmental levels. Without ECG-gating, contiguous slices of interesting parts of the cardiovascular system can be obtained and successfully used for secondary angiographic reconstructions

Abstract: Thirty-nine male patients (average age 50.8 years +/- 8.4 years) with a large anterior myocardial infarction (average 45.6 days +/- 10.5 days ago) and with moderate to severe left- ventricular dysfunction (RNVA EF < 50 %) participated in the study. The patients were randomly assigned to either a training group or to a control group. They were also subdivided into training/control groups (EF < 30 % and EF = 30- 50 %). The training program consisted of three to four sessions per day, 5 days a week, at an intensity of up to 1 W/kg body wt. (approximately 4-5 METS). The following evaluations were recorded prior to and following the 4-week training program: relative heart volume (x-ray), echocardiographic data (enddiastolic diameter, ES-distance, and shortening fraction), and exercise stress test (work capacity, heart rate). Filling pressures, cardiac outpout, and stroke volume index were calculated from right-heart catheterization (Swan-Ganz) at rest and during exercise. Results indicate that there were no significant changes in relative heart volume, end-diastolic volume, ES-distance, resting heart rate, PCP at rest, and ejection fraction during exercise as a result of the training program. Shortening fraction showed a tendency to improve (not significant). Work capacity increased by 15 W (p < 0.05) in the training group and by 28 W (1.5 METS, p < 0.05) in the EF < 30 % training-group as compared with the control group. Cardiac output at rest decreased by 10 % (p < 0.05). Stroke- volume index increased in the EF > 30 % training-group, while heart rate was reduced. In the control-group, PCP at exercise increased while EF at rest showed a tendency to increase slightly (no significant). Severe complications occurred in four patients during the course of the program: all patients were in the training-group, three patients developed left-heart failure, and 1 patient developed ventricular tachycardia, necessitating defibrillation. These four patients were characterized by the pre-training parameters: age > 50 years; PCP (rest) > 15 mm Hg; PCP (max-ex) > 30 mm Hg; PWP < 50 W; EF (ex) < 20 %; ES-distance > 1.2 cm; frequent arrhythmias (Holter) Grade > III b Lown. Two out of four patients, who developed severe complications, had a left-ventricular aneurysm. Patients exhibiting these characteristics should be exercised only under close medical supervision. They definitely should not be permitted to participate in a vigorous exercise program, particularly if a left-ventricular aneurysm exists. Furthermore, 98 patients with large anterior-wall infarctions were subdivided into a group performing heavy- training and into a group performing light-training assignments and scored accordingly. The score used non-invasive methods, which are widely used in cardiology. When this code was used, complications during the 4-weeks training-period did not occur

Abstract: In 16 patients with sequential pacing and partly impaired left- ventricular function (mean shortening fraction 23 +/- 7 %), the flow through the mitral and aortic valves were studied with Doppler echocardiography at different AV-intervals, keeping heart rate constant at 70/min. The time-velocity integral of the early and late diastolic filling, the first one-third filling fraction, and the diastolic-filling time were found to be significantly dependent on the length of the AV interval (p < 0.001). In the left ventricular outflow tract, the maximum stroke volume of 85.6 +/- 28.4 ml was measured at an AV interval of 200 ms. The stroke volume under VOO stimulation (70.5 +/- 24.1 cm) was 17% less (p < 0.003). There was no correlation between the increase in stroke volume during sequential pacing and M mode or Doppler echocardiographic parameters. Thus, the haemodynamic benefit of sequential pacing cannot be predicted by echocardiography. Doppler echocardiography is useful for determining the AV interval leading to the maximal cardiac output at rest. In the Doppler echocardiographic assessment of left-ventricular function, the influence of the AV interval on diastolic-filling parameters must be considered
