Abstract: In a study sample consisting of 388 unselected, consecutive acute stroke patients, 27 with systolic blood pressure-greater- than-or-equal-to-200 mmHg and diastolic blood pressure-greater- than-or-equal-to-115 mmHg were compared with the other 361 patients. The patients with high blood pressure were younger (65 vs. 73 years) and much more often had a history of hypertension (78 vs. 42%). Cardiac and vascular hypertensive manifestations were more frequent, particularly when only those patients with a history of hypertension were compared in the two groups. Alcohol abuse was mentioned in a higher proportion of hypertensives in the summaries of their medical records. No definite conclusions could be drawn with regard to the size and location of the brain lesions. Clinical symptoms did not differ between the groups, neither did the proportion of patients who could be discharged from hospital immediately. Mortality was higher in the high blood pressure group (30 vs. 14%, P < 0.05). Thus the characteristics of patients with very high blood pressure were: younger age, much more frequent and severe previous hypertension. Alcohol abuse might be an important factor. The type, size and location of the brain lesion itself could not be statistically related to the high blood pressure, but very large lesions, particularly haemorrhages, might be associated with a reactive blood pressure response

Abstract: Methodologies have been developed and implemented in LISP and OPS-5 languages which address type synthesis of mechanisms. Graph theory and separation of structure from function concepts have been integrated into an expert system called DOMES (Design Of Mechanism by an Expert System) to effectively implement the following three activities: (1) enumeration of all nonisomorphic labelled graphs; (2) identification of those graphs which satisfy structural constraints; (3) sketching of a mechanism corresponding to a given graph. Developed theories and algorithms are applied to a Robot Gripper design [19] and a Variable Stroke Piston Engine design [16]. The results from these two applications indicate that the automated techniques effectively identify all previously obtained solutions via manual techniques. Additional solutions are also identified and several errors of the manual process are detected. The developed methodologies and software appear to perform a complete and unbiased search of all possible candidate designs and are not prone to the errors of the manual process. Other important features of DOMES are: (1) it can learn and reason, by analogy, about a new design problem based on its experience of the problems previously solved by the system; (2) it has the capability to incrementally expand its knowledge base of rejection criteria by converting into LISP code information obtained through a query-based interactive session with a human designer; (3) it can select the set of rejection criteria relevant to a design problem from its knowledge base of rejection criteria. These procedures could become a powerful tool for design engineers, especially at the conceptual stage of design

Abstract: Life events and difficulties were recorded for the year before stroke, using a standardised semi-structured interview, in 113 surviving patients seen after their first ever in a lifetime stroke. An age and sex-matched control group (n = 109) was also interviewed about the preceding year. The stroke patients reported fewer non-threatening events and events with only a short-term threat, while difficulties were reported with equal frequency by the two groups. However, events which were severely threatening in the long-term were significantly more common in the stroke patients (in the 52 weeks before stroke 26% versus 13%, odds ratio 2.3, 95% confidence interval 1.1- 4.9). The increased rate was apparent throughout the year and not just in the weeks immediately before stroke onset. The number of stroke patients experiencing severe events in the follow up year fell to the level found in the control group. Recognised risk factors for stroke were found equally in those patients with and without severe events before onset, except that hypertension was rather less common in the patients who had experienced a severe event. It therefore appears that severe life events may be one of the determinants of stroke onset

Abstract: Early diagnosis of acute cerebral ischaemia is still unsatisfactory, because X-ray computed tomography (CT) does not reveal the site and extent of hypoperfusion within the first 24h. Single photon emission computed tomography (SPECT) using (99m)Tc-hexamethyl-propylene amine oxime (HMPAO) may offer earlier information, since the distribution of HMPAO follows the actual cerebral perfusion pattern. We therefore investigated 53 patients suffering from acute cerebral ischaemia (10 with transient ischaemic attacks, 9 with prolonged ischaemic reversible neurological deficits, 34 with completed stroke). SPECT and CT examinations were performed on days 1, 3, and 14. On day 1, SPECT revealed hypoperfused areas in 42 patients, whereas CT showed hypodensities only in 5. The sensitivity of SPECT was higher in cortical compared with subcortical ischaemia. In patients suffering from reversible neurological deficits SPECT normalized in the follow-up, corresponding to clinical improvement. In completed stroke, SPECT demonstrated variable perfusion patterns with hypo-, normo-, and hyperperfused areas on day 3 and especially on day 14. In contrast to CT, HMPAO SPECT leads to early diagnosis of cerebral ischaemia, in particular within the cerebral cortex

Abstract: Forty six patients aged 18-39 years with transient ischaemic attacks (TIA) were studied; two thirds were women. Twenty five patients had attacks accompanied by headache, and seven gave a history of common migraine. Only four of 27 angiograms were abnormal; no operable carotid lesion was demonstrated. Over a mean follow up period of 10 years stroke or myocardial infarction (AMI) occurred in all four patients who presented major cerebrovascular risk factors, but in only two of the remaining 42 patients. Thus irrespective of age thromboembolic TIA is a harbinger of stroke or AMI. However, most TIAs under the age of 40 years are caused by a non-embolic benign vascular disorder. The clinical characteristics, long-term prognosis, and possible pathogenesis, for such attacks are often indistinguishable from those of classical migraine. In the absence of cardiovascular risk factors, arteriography does not provide much diagnostic and prognostic information
