Abstract: The present randomized primary prevention study in hypertensive men aged 40-64 years (n = 3,234) was aimed at investigating whether metoprolol given as initial treatment would prevent coronary artery disease (CAD) better than thiazide diuretics. Two hundred fifty-five patient-years of follow-up; 25% of these events were fatal, and 38% were definite acute myocardial infarctions. The incidence of CAD was significantly lower during follow-up in patients randomized to metoprolol than in patients randomized to diuretics: 111 vs. 144 cases (p = 0.001). Stroke mortality was significantly lower in the metoprolol group than in the diuretic group, but the overall stroke incidence was similar in the two treatment groups. A majority of events occurred among smokers in both treatment groups although only one-third of patients were smokers at baseline. Blood pressure (BP) control was similar in the two treatment groups; therefore, the difference between the groups in CAD events is mediated via mechanisms other than the BP- reducing effect of metoprolol

Abstract: The hemodynamic effects of nicardipine were studied in 10 normotensive patients (nine men, one woman; age 43-70 years, mean 56 years) and five patients with mild to moderate hypertension (four men, one woman; age 46-72 years, mean 62.8 years); in all patients coronary heart disease was confirmed by angiography. Hemodynamic parameters were determined before and after intravenous administration of nicardipine in cumulative doses of 2.5, 7.5, and 12.5 mg for 10 min each. Nicardipine significantly reduced systolic aortic pressure, diastolic aortic pressure, and mean aortic pressure in normotensive patients [139 +/- 8.7 vs. 114 +/- 9.2 mm Hg (p < 0.001), 73 +/- 9.1 vs. 56 +/- 7.9 mm Hg (p < 0.001), 97 +/- 7.8 vs. 78 +/- 7.9 mm Hg (p < 0.001), respectively] and in hypertensive patients [166 +/- 7.4 vs. 128 +/- 8.6 mm Hg (p < 0.001), 83 +/- 9.4 vs. 56 +/- 10.9 mm Hg (p < 0.001), 110 +/- 15.5 vs. 78 +/- 12.0 mm Hg (p < 0.001, respectively]. Systemic vascular resistance was decreased significantly in hypertensive patients [1,363 +/- 188 vs. 707 +/- 137.7 dyn sec cm-5 (p < 0.001)] and in normotensive patients [1,110 +/- 225.3 vs. 682 +/- 92.4 dyn sec cm-5 (p < 0.001)]. Heart rate increased from 68 +/- 6.8 to 83 +/- 11.4 beats/min (p < 0.001) in normotensive patients but the increase from 72 +/- 14.7 to 80 +/- 14.2 beats/min in hypertensive patients was not significant. Mean pulmonary artery pressure did not change significantly in hypertensive patients and increased slightly in normotensive patients. Pulmonary wedge pressure remained almost constant. Stroke volume index increased in normotensive patients from 50.5 +/- 7.4 to 56 +/- 3.9 ml/m2 (p < 0.01) and in hypertensive patients from 48 +/- 11 to 60 +/- 10 ml/m2 (p < 0.06), dp/dt(max) did not change in hypertensive patients and showed a slight increase in normotensive patients [1,502 +/- 226.3 vs. 1,884 +/- 402.7 mm Hg/s (p < 0.001)]. Improvement of wall motion abnormalities was observed in five normotensive patients and in two hypertensive patients. The preliminary results of Doppler echocardiographically assessed diastolic function measured in five patients did not show a uniform tendency. Side effects were observed in three patients (angina pectoris in one, headache in two). It is concluded that nicardipine provides potent vasodilator effects and can improve left ventricular function in hypertensive and normotensive patients with coronary heart disease

Abstract: This paper describes a basic investigation of possible non- response bias in a mail survey. We compare characteristics of responders and non-responders to a mail survey of health outcomes among participants of a longitudinal study of physical activity, physical fitness, and health. Results indicate that, at the first clinic visit, the responders were essentially the same as the non-responders on personal health history and laboratory measurements, while reporting significantly more family history of specific chronic diseases (cardiovascular disease, hypertension, stroke). The male responders were younger and reported more positive health behaviors as well as better weight and treadmill times at the first clinic visit. These results suggest that both response groups were equally healthy at entry, and that individuals who had family members with certain chronic conditions and who had positive health behaviors were more likely to respond (participate) in this health-related survey. Differences of this type could affect interpretation of future analyses. This work illustrates the importance of incorporating methods to examine non-response into any epidemiologic study

Abstract: Cardiac output was measured by the thermodilution method in three young harbor seals, at rest and while swimming up to the maximum effort for which they could be trained. Stroke volume was determined by counting heart rate simultaneously with determination of cardiac output. Cardiac outputs varied widely between surface breathing (7.8 ml.kg-1.s-1) and breath-holding while swimming under water (1.8 ml.kg-1.s-1). Stroke volume while at the surface was almost twice the volume while submerged. Surface cardiac output was always near maximal despite work effort, whereas submerged cardiac output gradually increased at higher work efforts. The cardiovascular performance of seals at the maximum MO2 we could induce from them is equivalent to that of the domestic goat

Abstract: Extensive reliance on manual controller tuning in closed-loop material testing applications makes such applications good candidates for self-tuning control implementation. The current work deals with the modeling, on-line identification, and self- tuning control of electrohydraulic servomechanisms used for closed-loop control of stroke, load, or strain involving low signal frequencies. The application to load and strain control results in servovalve operation close to null and increases the significance of nonideal aspects of servovalve operation such as valve mechanical null offset, valve overlap and radial clearance. Modifications in process modeling and on-line identification necessary to accommodate these aspects are described and shown to be effective by simulation and experiment. A pole-placement controller design approach is used for controller adaptation, the control algorithm being chosen based on analysis of robustness of the control system. The self-tuning control system is shown to be effective, by simulation and experiment
